Abstract Objective To construct and validate a synchronous and observational telesimulation design in nursing for care according to in-hospital basic life support protocol for adults. Methods A methodological study based on the phases of the theoretical procedure carried out through a scoping review; empirical phase developed through the telesimulated design content validity and analytical phase in which the content validity index was adopted. Results A care design was constructed according to the basic life support protocol, going through six steps: planning, preparation, participation, teledebriefing, assessment and feedback and additional learning, with a Content Validity Index of 0.96. Conclusion The developed design was considered valid in content to plan and execute telesimulation by nursing and still be adapted to other educational contexts.

Resumen Objetivo Elaborar y validar un diseño de telesimulación sincrónica y observacional de enfermería para la atención de acuerdo con el protocolo de soporte vital básico intrahospitalario en adultos. Métodos Estudio metodológico basado en las fases del procedimiento teórico realizado por medio de una scoping review, fase empírica llevada cabo a través de la validación de contenido del diseño telesimulado y fase analítica en la que se adoptó el índice de validez de contenido. Resultados Se elaboró un diseño para la atención de acuerdo con el protocolo de soporte vital básico que atravesó seis etapas: planificación, preparación, participación, teledebriefing, evaluación y feedback y aprendizaje adicional, con un Índice de Validez de Contenido de 0,96. Conclusión El diseño elaborado fue considerado válido en contenido para planificar y ejecutar la telesimulación por parte de enfermeros y además puede adaptarse a otros contextos educativos.

Resultados Construiu-se um design para o atendimento segundo o protocolo de suporte básico de vida percorrendo seis etapas: o planejamento, preparação, participação, teledebriefing, avaliação e feedback e a aprendizagem adicional, com Índice de Validade de Conteúdo de 0,96. Abstract The environmental fragility of the Amazon River Delta has been accentuated by urban growth. However, in an urban resilience point of view, it is clear that these cities have a capacity to reinvent themselves and overcome the low capacity of social investments. In this sense, this paper aims to present a discussion on the resilience of small towns in the Amazon River Delta, based on the analysis of three components: infrastructure, urban economy and social aspects. The research was developed using satellite images, data from the 2010 Brazilian Census and information collected in the field, through the application of questionnaires to 874 households of three small cities: Afuá, Ponta de Pedras, located at the state of Pará, and Mazagão, in the state of Amapá. The study showed that the resilience of small cities can be seen as the ability to overcome structural problems and present possibilities for the resident population to reinvent themselves.

Resumo A fragilidade ambiental do delta do rio Amazonas se acentua em função do crescimento urbano. A partir de uma discussão sobre resiliência urbana, percebe-se que as cidades no entorno têm competência para se reinventar e superar a baixa capacidade de investimentos sociais. Nesse sentido, este artigo tem como objetivo discutir sobre a resiliência das pequenas cidades desse delta, mediante a análise de três componentes: infraestrutura, economia urbana e aspectos sociais. A pesquisa foi elaborada com a utilização de imagens de satélite, dados do Censo de 2010 e informações coletadas em campo, por meio da aplicação de questionários a 874 domicílios de três pequenas cidades: Afuá, Ponta de Pedras e Mazagão. O estudo demonstrou que a resiliência das pequenas cidades é capaz de superar os problemas estruturais e apresentar possibilidades para a população moradora se reinventar.

Abstract To compare the prevalence of hospitalizations and deaths due to Leishmaniasis among Brazilian regions from 2010 to 2020. This is a retrospective, descriptive, quantitative, documentary-based study. Data were obtained from the SUS Hospital Information System (SIH/SUS), by the SUS IT Department (DATASUS). In the period evaluated, 32.320 hospitalizations for Leishmaniasis were recorded in Brazil. Visceral leishmaniasis comprised the majority of hospitalizations 25.761 (79.70%). However, the highest numbers of hospitalizations occurred in the Northeast (42.82%) and Southeast (19.91%) for visceral leishmaniasis, Southeast (37.29%) and Northeast (33.42%) for Cutaneous Leishmaniasis and Southeast (42.82%) and Northeast (22.94%) due to mucocutaneous leishmaniasis. There was a predominance of males and brown color/race. A total of 1.193 deaths were observed. The highest number of deaths occurred in patients hospitalized for visceral leishmaniasis (1.086/91.03%). Leishmaniasis and, consequently, hospitalizations and deaths due to this pathology, cause a considerable impact on the public health system in Brazil, generating high costs for the country. Thus, government measures are needed to stimulate health promotion actions for disease prevention.

Resumo Comparar as prevalências das internações e óbitos por Leishmaniose entre as regiões brasileiras no período de 2010 a 2020. Trata-se de um estudo retrospectivo, descritivo, quantitativo, de base documental. As variáveis utilizadas foram: sociodemográficas (sexo, faixa etária e etnia) e clínicas (número de internações e média de permanência, taxa de mortalidade, caráter, regime de atendimento e custos das internações). Os dados foram obtidos do Sistema de Informações Hospitalares do SUS (SIH/SUS), pelo Departamento de informática do SUS (DATASUS). No período avaliado, foram registradas 32.320 internações por Leishmaniose no Brasil. A Leishmaniose visceral compondo a maior parte das internações 25.761 (79.70%). No entanto, os maiores números de internações ocorreram nas regiões Nordeste (42.82%) e Sudeste (19.91%) pela Leishmaniose visceral, Sudeste (37.29%) e Nordeste (33.42%) na Leishmaniose Cutânea e Sudeste (42.82%) e Nordeste (22.94%) pela Leishmaniose mucocutânea. Houve predomínio no sexo masculino e cor/raça parda. Observou-se um total de 1.193 óbitos. O maior número de óbitos ocorreu nos pacientes internados por Leishmaniose visceral (1.086/91.03%). As Leishmanioses e, consequentemente as internações e óbitos em decorrência dessa patologia, ocasiona considerável impacto no sistema público de saúde do Brasil, gerando altos custos para o país. Dessa forma, são necessárias medidas governamentais, com objetivo de estimular ações de promoção de saúde prevenção de doenças.

Abstract Objective The aim of this study was to analyze the effects of fluid overload related to mechanical ventilation, renal replacement therapy, and evolution to discharge or death in critically ill children. Methods A retrospective study in a Pediatric Intensive Care Unit for two years. Patients who required invasive ventilatory support and vasopressor and/or inotropic medications were considered critically ill. Results 70 patients were included. The mean age was 6.8 ± 6 years. There was a tolerable increase in fluid overload during hospitalization, with a median of 2.45% on the first day, 5.10% on the third day, and 8.39% on the tenth day. The median fluid overload on the third day among those patients in pressure support ventilation mode was 4.80% while the median of those who remained on controlled ventilation was 8.45% (p = 0.039). Statistical significance was observed in the correlations between fluid overload measurements on the first, third, and tenth days of hospitalization and the beginning of renal replacement therapy (p = 0.049) and between renal replacement therapy and death (p = 0.01). The median fluid overload was 7.50% in patients who died versus 4.90% in those who did not die on the third day of hospitalization (p = 0.064). There was no statistically significant association between death and the variables sex or age. Conclusions The fluid overload on the third day of hospitalization proved to be a determinant for the clinical outcomes of weaning from mechanical ventilation, initiation of renal replacement therapy, discharge from the intensive care unit, or death among these children.

Resumo Estratégias de buscas mal formuladas podem apresentar grandes influências nos resultados de uma metanálise, uma vez que impacta diretamente na quantidade e aderência ao tema dos trabalhos utilizados para estudo, portanto, a formulação de uma estratégia de busca consistente e funcional é fundamental para que a revisão sistemática atinja seus objetivos. O objetivo deste trabalho foi estudar o impacto de diferentes estratégias de busca em uma revisão sistemática por meio da realização de uma metanálise para estimação de herdabilidade para a característica mastite em gado de leite. Uma vez definidas as estratégias de busca, as pesquisas realizadas nas bases de dados Web of science, Scopus, Scielo e Pubmed retornaram 921 estudos dos quais, após passarem pelos processos de identificação, seleção, elegibilidade e inclusão, 25 estudos foram selecionados. Retiradas dos artigos selecionados, 26 estimativas de herdabilidades foram utilizadas na realização da metanálise. Utilizou-se um modelo de efeito aleatório, sendo todas as análises realizadas pelo programa R, por meio do pacote Metafor. As estimativas obtidas através da estatística combinada de estudos para mastite, apresentou valores de baixa magnitude (0,05 e 0,06). Os efeitos das estratégias de busca têm impacto significativo nas estimativas de metanálise produzidas.

Abstract Poorly formulated search strategies can have great influences on the results of a meta-analysis, since it directly impacts the amount and adherence to the theme of the works used for study, therefore, the formulation of a consistent and functional search strategy is essential for the review system to achieve its goals. The objective of this work was to study the impact of different search strategies in a systematic review by performing a meta-analysis to estimate heritability for the mastitis trait in dairy cattle. Once the search strategies were defined, the searches carried out in the Web of science, Scopus, Scielo and Pubmed databases returned 921 studies from which, after going through the identification, selection, eligibility and inclusion processes, 25 studies were selected. Withdrawals from selected articles, 26 heritability estimates were used in the meta-analysis. A random effect model was used, with all analyzes performed by the R program, through the Metafor package. The estimates obtained through the combined statistics of studies for mastitis, presented values of low magnitude (0.05 and 0.06). The effects of search strategies have a significant impact on the meta-analysis estimates produced.

ABSTRACT Objective: To assess the in-vitro effect of single applications of CPP-ACP pastes and different fluoridated solutions on the prevention of dental caries around orthodontic brackets. Material and Methods: Tooth/bracket sets (n=65) were immersed in artificial saliva (1h at 37ºC) and randomly subjected to single applications (100µL; 1min) of casein phosphopeptide-amorphous calcium phosphate (CPP-ACP emulsion), CPP-ACP with fluoride (CPP-ACPF emulsion), solutions of titanium tetrafluoride (TiF4) or sodium fluoride (NaF), or no treatment (CG). Multispecies biofilm (5 x 105 CFU/mL) was formed in the presence of 2% sucrose. After 24 h, the pH and the concentration of total soluble fluoride (TSF) were analyzed by culture medium. The presence of active white spot lesions (WSL) evaluated by macroscopic examination and the percent surface mineral loss (%SML) were analyzed. Also, the topography of enamel was detected by analysis of scanning electron microscopy (SEM). The data was assessed by chi-square, Kruskal-Wallis, and Mann-Whitney tests (p < 0.05). Results: Fluoride-containing compounds led to a smaller pH reduction than did CPP-ACP and CG (p<0.05). There was difference in TSF between the groups (p<0.05), denoted as TiF4> NaF > CPP-ACPF > CPP-ACP > CG. Regarding the presence of WSL and %SML, the NaF group obtained lower values (p<0.05), while TiF4 and CPP-ACPF were similar (p>0.05). SEM demonstrated that fluoride-free groups had a larger surface dissolution. Conclusion: Fluoridated groups including solutions and CPP-ACPF were more effective than CPP-ACP in reducing enamel demineralization around orthodontic brackets after a single application.

RESUMO Objetivo: Avaliar in-vitro o efeito de uma aplicação única de cremes dentais de CPP-ACP e diferentes soluções fluoretadas na prevenção da cárie dentária ao redor de braquetes ortodônticos Material e Métodos: O conjunto dentes/braquetes (n=65) foi imerso em saliva artificial (1h em 37°C) randomizado e submetido a tratamento único (100µL; 1 min) de emulsão de fosfopeptídeo de caseína-fosfato de cálcio amorfo (CPP-ACP) e CPP-ACP associado ao flúor (CPP-ACPF); soluções de tetrafluoreto de titânio (TiF4) e fluoreto de sódio (NaF); e ausência de tratamento (GC). Biofilmes multiespécie (5 x 105 CFU/mL) foram formados na presença de sacarose a 2%. Após 24h, o pH e a concentração de fluoreto solúvel total (FST) foram analisados pelo meio de cultura. Foram avaliadas a presença de lesões de mancha branca (LMB), por meio da análise de macroscopia visual, e a porcentagem de perda de dureza (%PD). Também foi verificada a topografia do esmalte, usando microscopia eletrônica de varredura (MEV). Os dados foram analisados pelos testes Qui-quadrado, Kruskal-Wallis e Mann-Whitney (p < 0,05). Resultados: Os compostos contendo flúor levaram a uma redução do pH menor do que o CPP-ACP e GC (p<0,05). Houve diferença no FST entre os grupos (p <0,05), sendo TiF4> NaF > CPP-ACPF > CPP-ACP > GC. Quanto à presença de LMB e à %PD, o grupo NaF obteve os menores valores (p<0,05), enquanto TiF4 e CPP-ACPF foram semelhantes (p> 0,05). A MEV demonstrou que os grupos sem flúor tiveram uma dissolução superficial maior. Conclusão: Os grupos fluoretados, incluindo soluções e CPP-ACPF, foram mais eficazes do que o CPP-ACP sem flúor na redução da desmineralização do esmalte ao redor dos braquetes ortodônticos após uma única aplicação.

ABSTRACT BACKGROUND: The incidence of esophageal cancer is high in some regions and the surgical treatment requires reference centers, with high volume, to make surgery feasible. AIMS: To evaluate patients undergoing minimally invasive esophagectomy by thoracoscopy in prone position for the treatment of esophageal cancer and to recognize the experience acquired over time in our service after the introduction of this technique. METHODS: From January 2012 to August 2021, all patients who underwent the minimally invasive esophagectomy for esophageal cancer were retrospectively analyzed. In order to assess the factors associated with the predefined outcomes as fistula, pneumonia, and intrahospital death, we performed univariate and multivariate logistic regression analyses, accounting for age as an important factor. RESULTS: Sixty-six patients were studied, with mean age of 59.5 years. The main histological type was squamous cell carcinoma (81.8%). The incidence of postoperative pneumonia and fistula was 38% and 33.3%, respectively. Eight patients died during this period. The patient's age, T and N stages, the year the procedure was performed, and postoperative pneumonia development were factors that influenced postoperative death. There was a 24% reduction in the chance of mortality each year, associated with the learning curve of our service. CONCLUSIONS: The present study presented the importance of the team's experience and the concentration of the treatment of patients with esophageal cancer in reference centers, allowing to significantly improve the postoperative outcomes.

RESUMO RACIONAL: A incidência do câncer de esôfago é elevada em algumas regiões e o tratamento cirúrgico requer centros de referência com alto volume para viabilizar a cirurgia. OBJETIVOS: Avaliar os pacientes submetidos à esofagectomia minimamente invasiva, por toracoscopia na posição prona, para o tratamento do câncer de esôfago e conhecer a experiência adquirida ao longo do tempo em nosso serviço após a introdução desta técnica. MÉTODOS: De janeiro de 2012 a agosto de 2021, foram analisados retrospectivamente todos os pacientes submetidos à esofagectomia minimamente invasiva para câncer de esôfago. Para avaliar os fatores associados aos desfechos predefinidos de fístula, pneumonia e óbito intra-hospitalar, realizamos análises de regressão logística univariada e multivariada considerando a idade como fator importante. RESULTADOS: Foram estudados 66 pacientes, com idade média de 59,5 anos. O tipo histológico mais frequente foi carcinoma espinocelular (81,8%). A incidência de pneumonia pós-operatória e fístula foi de 38% e 33,3%, respectivamente. Oito pacientes morreram durante este período. A idade do paciente, os estágios T e N, o ano da realização do procedimento e o desenvolvimento de pneumonia pós-operatória foram fatores que influenciaram o óbito. Houve uma redução de 24% na chance de mortalidade, ano a ano, associada à curva de aprendizado do nosso serviço. CONCLUSÕES: O presente estudo mostrou a importância da experiência da equipe e da concentração do tratamento de pacientes com câncer de esôfago em centros de referência, possibilitando melhorar significativamente o resultado pós-operatório.

Abstract Inducible nitric oxide synthase (iNOS) is one of the enzymes responsible for the synthesis of nitric oxide (NO), which is an important signaling molecule with effects on blood vessels, leukocytes, and bone cells. However, the role of iNOS in alveolar bone healing remains unclear. This study investigated the role of iNOS in alveolar bone healing after tooth extraction in mice. Methodology C57Bl/6 wild type (WT) and iNOS genetically deficient (iNOS-KO) mice were subjected to upper incision tooth extraction, and alveolar bone healing was evaluated by micro-computed tomography (μCT) and histological/histomorphometric, birefringence, and molecular methods. Results The expression of iNOS had very low control conditions, whereas a significant increase is observed in healing sites of WT mice, where iNOS mRNA levels peak at 7d time point, followed by a relative decrease at 14d and 21d. Regarding bone healing, both WT and iNOS-KO groups showed the usual phases characterized by the presence of clots, granulation tissue development along the inflammatory cell infiltration, angiogenesis, proliferation of fibroblasts and extracellular matrix synthesis, bone neoformation, and remodeling. The overall micro-computed tomography and histomorphometric and birefringence analyses showed similar bone healing readouts when WT and iNOS-KO strains are compared. Likewise, Real-Time PCR array analysis shows an overall similar gene expression pattern (including bone formation, bone resorption, and inflammatory and immunological markers) in healing sites of WT and iNOS-KO mice. Moreover, molecular analysis shows that nNOS and eNOS were significantly upregulated in the iNOS-KO group, suggesting that other NOS isoforms could compensate the absence of iNOS. Conclusion The absence of iNOS does not result in a significant modulation of bone healing readouts in iNOS-KO mice. The upregulation of nNOS and eNOS may compensate iNOS absence, explaining the similar bone healing outcome in WT and iNOS-KO strains.

ABSTRACT The aim of this study was to report the clinical case of an 8-year-old male patient seen at the Pediatric Dentistry outpatient clinic at the Universidade Federal do Paraná, Brazil, diagnosed with molar-incisor hypomineralization (MIH) after presenting with pain in teeth 16, 36, and 46, which had extensive atypical cavities and opacities on the adjacent enamel. A diagnostic wax-up of affected teeth was performed for manufacture of silicone occlusal matrix. Encapsulated high-viscosity glass ionomer cement was handled and inserted into the occlusal matrix which was fitted onto the teeth by slight finger pressure. After setting time, excess of material was removed, and occlusion was checked. A flowable resin coating material was applied, and the restorations were finished. Six months later, the restorations performed for treatment of the severe MIH lesions proved to be efficient, lowering the sensitivity reported by the patient, reshaping the teeth, improving their function, and refining dental esthetics.

RESUMO O objetivo deste estudo foi relatar um caso clínico de um paciente com diagnóstico de Hipomineralização de Molares e Incisivos (HMI). Paciente do sexo masculino, de 8 anos de idade, compareceu à clínica de Odontopediatria da Universidade Federal do Paraná com queixa de dor nos dentes 16, 36 e 46 - os quais apresentavam extensas cavitações atípicas com opacidades no esmalte adjacente. O enceramento diagnóstico dos dentes afetados foi realizado para a confecção das matrizes oclusais de silicone. Cimento ionômero de vidro de alta viscosidade encapsulado foi manipulado e inserido na matriz oclusal, a qual foi encaixada sobre os dentes, exercendo-se leve pressão digital. Após a presa do material os excessos foram removidos e a oclusão foi checada. Aplicou-se uma camada de resina fluída de cobertura e as restaurações foram finalizadas. Após seis meses, as restaurações realizadas para tratar as lesões severas de HMI mostraram-se eficazes, reduzindo a sensibilidade relatada pelo paciente, além de reestabelecerem a anatomia, função e estética dentária.

ABSTRACT Nitrogen (N) fertilizer management is a key factor in the success of maize crops. The objective of this work was to evaluate the response of second-crop maize to N applied exclusively as topdressing or at planting + topdressing. Five experiments were conducted in areas in southwestern Goiás in 2019 and 2020 to evaluate N application rates of 0 and 30 kg ha-1 at planting, combined with five N rates applied as topdressing at the maize V3 stage over a total of 0, 75, 150, 225, and 300 kg ha-1. Dry matter production in the V6 or V9 stage and yield parameters at harvest were evaluated. N application did not affect the number of grain rows per ear but increased the number of grains per row and, particularly, grain weight. The crop responded positively to increases in the total N rate in 4 of the 5 experiments: in 1 of these experiments, N fertilizer application at planting contributed to an increase in the efficiency of topdressing fertilization, resulting in a higher maize yield with a lower total N rate; in the other 3, a positive maize response to N occurred when total N was applied as topdressing at V3. However, the highest mean maize grain yield (8,233 kg ha-1) was found in the trial that showed no response to N fertilizer application - probably due to the better chemical fertility conditions of the area combined with an adequate distribution of rainfall, considering the multiple factors involved in N application recommendations.

RESUMO O manejo de nitrogênio (N) na cultura do milho é fator chave para o sucesso de seu cultivo, de modo que se objetivou com esse estudo avaliar a resposta da cultura, em segunda safra, à doses de N aplicadas exclusivamente em cobertura ou na adubação de base (semeadura) complementada com a adubação de cobertura. Para tal, em 2019 e 2020 foram realizados cinco experimentos em áreas da região Sudoeste do Estado de Goiás, testando 0 e 30 kg ha-1 de N aplicados na base combinados com cinco doses totais de N (0, 75, 150, 225 e 300 kg ha-1) completadas em cobertura, quando as plantas estavam em estádio V3. Avaliou-se a produção de massa seca na fase vegetativa V6 ou V9, e, por ocasião da colheita, os parâmetros produtivos. Verificou que a aplicação de N não interfere no número de fileiras de grãos, mas aumenta o número de grãos por fileira e, principalmente, o peso de grãos. Em 4 dos 5 ensaios, a cultura respondeu positivamente ao incremento da dose total de N; em 1 desses, a adubação de base contribuiu para aumento da eficiência da adubação de cobertura, de modo que a produtividade do milho foi maior e atingida com uma dose total de N menor. Nos outros 3, respostas positivas foram observadas quando da aplicação total do N em cobertura e em dose única. A maior produtividade média de grãos de milho (8.233 kg ha-1), no entanto, ocorreu justamente no ensaio com ausência de resposta à adubação nitrogenada - provavelmente devido às melhores condições de fertilidade química da área aliada à adequada distribuição das chuvas, considerando assim múltiplos fatores para N recomendações de aplicação.

Abstract Neurofibromatosis Type 1 (NF1) is a rare cause of ischemic stroke (IS) in the general population. We report a case of a young patient with NF1 in whom IS was caused by fibromuscular dysplasia. An angiographic study demonstrated occlusion in the right internal carotid artery (ICA), just after its origin, and the left ICA, just before the intracranial portion, and brain magnetic resonance imaging showed the limits of an area of brain infarction in the right frontoparietal region. Despite these concomitant neuroimaging findings, this association is rare, and it is difficult to establish the contribution to the outcome made by each of these diseases, which treatment is the best to implement, or what prognosis is.

Resumo A neurofibromatose tipo 1 (NF1) é uma causa rara de acidente vascular cerebral isquêmico (AVCi) na população geral. Neste estudo, relatamos o caso de um paciente jovem com AVCi, com diagnóstico de NF1 associada a displasia fibromuscular. O estudo angiográfico demonstrou oclusão da carótida interna direita, logo após sua origem, e esquerda, antes da porção intracraniana. A ressonância magnética do encéfalo mostrou delimitação de um infarto na região frontoparietal direita. Apesar desses achados concomitantes na neuroimagem, essa associação é rara, sendo difícil de estabelecer a contribuição de cada uma dessas doenças no desfecho, tampouco qual o melhor tratamento a ser implementado e qual o prognóstico.

ABSTRACT Objective To evaluate the accuracy of corneal tomography with Scheimpflug images (Pentacam® AXL, OCULUS, Wetzlar, Germany) in scanning patterns with 25 and 50 images, by analyzing their repeatability and reproducibility in normal eyes, eyes with keratoconus and with keratoconus after crosslinking. Methods Prospective, cross-sectional, mixed study including patients who had normal corneas, or who had keratoconus, who had not undergone corneal surgery, with the exception of crosslinking. Patients who had undergone surgery for intracorneal ring implantation or corneal transplantation were excluded. Three measurements were performed with 25 images and three measurements were performed with 50 images on each eye included in the study. Results The study evaluated a total of 54 eyes of 41 patients. Of these, 10 (18.52%) eyes had normal corneas, without previous eye surgery, 22 (40.74%) were eyes with keratoconus, without previous eye surgery, and 22 (40.74%) were eyes with keratoconus, with crosslinking for keratoconus. In the study of repeatability, we noticed a greater variation of values in all indices in patients with KC compared to those with normal eyes, in most of the parameters analyzed. Conclusion There is no significant difference between the scanning methods with 25 and 50 images. However, 25 scans has advantages that make it the main method suggested. The presence of keratoconus has a negative impact on the repeatability of both types of examination, and there is great agreement between the methods in the three groups studied. The high repeatability coefficient in some parameters suggests that these data should be used with care in the assessment of keratoconus progression.

RESUMO Objetivo Avaliar a precisão da tomografia corneana com imagens de Scheimpflug (Pentacam® AXL, OCULUS, Wetzlar, Alemanha) nos padrões de escaneamento com 25 e 50 imagens, verificando sua repetibilidade e reprodutibilidade em olhos normais, com ceratocone e com ceratocone após crosslinking. Métodos Estudo prospectivo, transversal, misto, no qual foram incluídos os pacientes que tinham córneas normais, ou com ceratocone; e não tinham realizado cirurgias corneanas, com exceção do crosslinking. Foram excluídos os pacientes que tivessem realizado outras cirurgias, como implante de anel intracorneano ou transplante de córnea. Foram realizadas três medidas com 25 imagens e três medidas com 50 imagens em cada olho incluído no estudo. Resultados O estudo avaliou 54 olhos de 41 pacientes. Destes, dez (18,52%) olhos eram com córneas normais e sem cirurgia ocular prévia aleatoriamente selecionados de 5 pacientes; 22 (40,74%) olhos aleatoriamente selecionados de 18 pacientes com ceratocone, sem cirurgia ocular prévia; e 22 (40,74%) olhos com ceratocone de 18 pacientes operados por crosslinking. No estudo da repetibilidade, percebemos uma maior variação de valores em todos os índices nos pacientes com ceratocone comparados aos de pacientes com olhos normais, em grande parte dos parâmetros analisados. Conclusão Não houve diferença significativa entre os métodos de escaneamento por 25 e 50 imagens, sendo o escaneamento por 25 imagens mais conveniente. A presença de ceratocone teve impacto negativo na repetibilidade de ambos os tipos de exame, e foi grande a concordância entre os métodos nos três grupos estudados. O coeficiente de repetibilidade permite considerar a relevância clínica dos dados para avaliar a progressão de ceratocone.

Abstract Objective To present the update of the recommendations of the Brazilian College of Radiology and Diagnostic Imaging, the Brazilian Society of Mastology and the Brazilian Federation of Associations of Gynecology and Obstetrics for breast cancer screening in Brazil. Methods Scientific evidence published in Medline, EMBASE, Cochrane Library, EBSCO, CINAHL and Lilacs databases between January 2012 and July 2022 was searched. Recommendations were based on this evidence by consensus of the expert committee of the three entities. Recommendations Annual mammography screening is recommended for women at usual risk aged 40–74 years. Above 75 years, it should be reserved for those with a life expectancy greater than seven years. Women at higher than usual risk, including those with dense breasts, with a personal history of atypical lobular hyperplasia, classic lobular carcinoma in situ, atypical ductal hyperplasia, treatment for breast cancer or chest irradiation before age 30, or even, carriers of a genetic mutation or with a strong family history, benefit from complementary screening, and should be considered individually. Tomosynthesis is a form of mammography and should be considered in screening whenever accessible and available.

Resumo Objetivo Apresentar a atualização das recomendações do Colégio Brasileiro de Radiologia e Diagnóstico por Imagem da Sociedade Brasileira de Mastologia e da Federação Brasileira das Associações de Ginecologia e Obstetrícia para o rastreamento do câncer de mama no Brasil. Métodos Foram pesquisadas evidências científicas publicadas nas bases de dados Medline EMBASE Biblioteca Cochrane EBSCO CINAHL e Lilacs entre janeiro de 2012 e julho de 2022. As recomendações foram baseadas nessas evidências por consenso do comitê de especialistas das três entidades. Recomendações A mamografia anual é recomendada para mulheres com risco habitual entre 40 e 74 anos. Acima de 75 anos deve ser reservado para aqueles com expectativa de vida superior a sete anos. Mulheres com risco maior do que o normal incluindo aquelas com mamas densas com história pessoal de hiperplasia lobular atípica carcinoma lobular in situ clássico hiperplasia ductal atípica tratamento para câncer de mama ou irradiação de tórax antes dos 30 anos ou ainda portadoras de doença genética mutação ou com forte histórico familiar beneficiam-se de triagem complementar e devem ser considerados individualmente. A tomossíntese é uma forma de mamografia e deve ser considerada na triagem sempre que acessível e disponível.
